Isosorbide

March 10, 2008

Pharmacological action of Isosorbide:

Peripheric vasodilator with primary influence on venous vessels. Mononitrate stimulates formation of nitric oxide in endothelium of vessels. Reduces requirement of a myocardium for oxygen at the expense of preload depression. Mononitrate also possesses coronarodilator action. Reduces blood inflow to the right auricle, promotes pressure decrease in a “small” circle of a circulation and regress of symptoms at edema of lungs. Isosorbide promotes redistribution of a coronary blood flow in area with the lowered circulation. Raises tolerance to physical loadings at sick of ischemic illness of heart, stenocardia. Dilates vessels of a brain, a firm cerebral cover, that can be accompanied by a headache. After a break in application of Isosorbide, the sensitivity to it is quickly restored. Antianginal effect comes after 30-45 minutes after intake and proceeds up to 8-10 hours.

Indications:

Stenocardia (preventive maintenance). Сhronic heart failure, a hypertensia in a “small” circle of circulation, “pulmonary” heart (treatment as a part of the combined therapy). A spastic stricture of peripheric arteries (an obliterating endarteritis, angiospastic retinitis).

Contraindications:

Hypersensitivity to Mononitrate.Apply Isosorbide with care to all forms of diseases (comparing risk and advantage), such as a hemorrhagic stroke, an acute myocardial infarction (risk of depression of a BP and a tachycardia, which can strengthen an ischemia), a glaucoma (risk of rising of an ophthalmotonus), serious anaemia, thyrotoxicosis, arterial hypotension with low systolic arterial pressure (can aggravate a condition, having caused a paradoxical bradycardia and stenocardia attacks), increase of attacks of stenocardia is possible, serious renal insufficiency, hepatic insufficiency (risk of development of ethemoglobinemia), pregnancy, the lactemia period, children’s age (safety of application is not established). For the prolonged medicinal forms for intake - the raised peristalsis of gastrointestinal tract, malabsorption syndrome.

By effects of
Isosorbide:

From cardiovascular system: headache, giddiness, dermahemia of the face, sensation of fever, tachycardia, BP depression. In rare cases - intensifying of attacks of stenocardia (paradoxical reaction) and a collapse. From the alimentary system: a nausea, vomiting, occurrence of sensation of an easy burning sensation of tongue, dryness in a mouth is possible. From nervous system: constraint, drowsiness, sight illegibility, depression of ability to fast mental and impellent reactions (especially in the beginning of the treatment). In rare cases - a brain ischemia. Allergic reactions: dermal eruption. Other: tolerance development (including two-dimensional to other Sodium nitritums), exfoliative dermatitis. An overdosage. Symptoms: collapse, syncopal conditions, headache, giddiness, palpitation, visual disorders, hyperthermia, cramps, dermahemia, the raised diaphoresis, nausea, vomiting, diarrhoeia, methemoglobinemia (cyanosis, anoxia - usually at chronic overdosage), hyperpnea, dyspnea, bradycardia, rising of intracranial pressure, paralysis, coma.
Way of application and dose:

Inside (tablets, capsules), 1 hour before or 2 hours after meal. Accept capsules, not chewing, washing down with a glass of water. An initial dose - 10-20 mg 2 times a day (for a medical product of average duration of action) or 40-50 mg 1 time a day (for capsules retard and depo tablets). Since 3-4 days it is possible to enlarge a dose on about 20-40 mg 2 times a day (for a medical preparation of average duration of action), enlarging a dose if necessary to 60-80 mg/day. Maximum daily dose of Isosorbide is 80 mg.

Special indications:

In therapy the control of BP and frequency of reductions of heart is necessary. In case of need to use Isosorbide against arterial hypotension it is necessary to apply simultaneously a medical preparation, possessing positive inotropic effect, or to apply artificial circulatory support agents. Frequent appointment and high doses of Isosorbide can cause tolerance development; in this case the cancellation for 24-48 hours is recommended or after 3-6 weeks of regular reception to do a break for 3-5 days, replacing on this time Isosorbidum Mononitrate with other antianginal medical preparations. It is necessary to avoid sudden cancellation of Isosorbide (reduce a dose gradually). In treatment it is necessary to be careful at driving and employment by other potentially dangerous kinds of the activity demanding raised concentration of attention and speed of psychomotor reactions.

Feldene

March 7, 2008

Feldene belongs to the group of preparations, which are called nonsteroid anti-inflammatory drugs, NSAID. These preparations are is often used for treatment of arthritis. Application of NSAID can have serious consequences for health up to a lethal outcome owing to a gastric or intestinal bleeding. The bleeding can arise at any time without any warning symptoms. Some doctors consider that to people more senior than 70 years it is better to begin treatment by preparations of this group with half of usual therapeutic dose.

Feldene stops pain and reduces inflammation caused by two kinds of arthritis - rheumatic arthritis and osteoarthritis. A significant number of serious by-effects and lethal outcomes, especially at older persons has been noted. At people more senior than 60 years the probability of bleeding in gastrointestinal tract, ulceration and perforation of wall of a stomach is higher, than at more young men. These negative effects took place, even when the patients did not exceed the prescribed dosages. Serious dermal diseases, up to lethal outcomes were observed in age groups.

Though Feldene is attractive by that it should be accepted only once a day, this “convenience” is rather disputable for older people. At this age group of people preparations are deduced from an organism more slowly so the big unitary dose can be deduced worse, than the small doses accepted during a day. As a result a dangerous level of concentration of this preparation in blood is formed.

After numerous reports on negative effects of this preparation at application of it by older people, Canadian drug regulatory agency recommended to people more senior than 65 years to begin treatment from 10 mg a day. It is a half of the dose recommended in the USA for application at adult patients.

All preparations of the NSAID group may cause damages of gastrointestinal tract, which may lead to bleeding in gastrointestinal tract and hospitalization or death. In process of augmentation of doses and duration of reception the probability of development of toxic effects is also enlarged.

If at reception of NSAID you will have the following symptoms, immediately stop reception of a preparation and address to the attending physician: strong pains in the field of a stomach, spastic strictures or burning, a strong and constant nausea, a heartburn, a dyspepsia, bloody or black stool, vomiting by blood or a coffee ground, and also blood spitting.

Indications to application:

Articular syndrome (pseudorheumatism / infectious-allergic diseases from the group of collagens, characterised by chronic progressing inflammation of joints/, osteoarthritis/inflammation of a joint and osteal tissue adjoining to it/, ankylosing spondylitis backbone/disease/, gout); pains in backbone, neuralgia (pain extending through a nerve), myalgia (pain in muscles), traumatic inflammation of soft tissues and locomotor apparatus; the primary dysmenorrhea (disorder of a menstrual cycle) at patients above 12 years; acute infectious-inflammatory diseases of the top respiratory tracts.

Inflammatory and degenerate diseases of locomotorium: rhematoid and gouty arthritis, rheumatic disease, ankylosing spondylarthritis (Bekhterev’s disease), osteoarthrosis. A painful syndrome: sciatica, tendovaginitis, bursitis, arthralgia, neuralgia, myalgia, radiculitis, the posttraumatic and postoperative painful syndrome accompanied by an inflammation, algodismenorrhea. Feverish syndrome at “catarrhal” and infectious diseases.

Side effects:

Disorders from gastroenteric tract (nausea, loss of appetite, pain and unpleasant sensations in the stomach, swelling of intestine, constipation, diarrhea) are possible, but in most cases they do not influence a treatment course. Cases of occurrence of erossive-ulcer damages (superficial defect of a mucous membrane), bleedings and perforations (occurrence of through defect in a wall of a genital organ) of gastroenteric tract are registered. Infringements of function of liver and/or kidneys are in rare instances possible; stomatitis (an inflammation of mucous membrane of an oral cavity), dermal rash and itch of allergic character; edemas of anticnemions and feet, giddiness, headache, drowsiness, sleeplessness, depression (depressive condition), irritability, hallucinations (delirium, visions getting character of a reality); changes of peripheric blood: anaemia (haemoglobin content depression in blood), leukopenia (depression of level of leucocytes in blood), thrombocytopenia (reduction of number of thrombocytes in blood).

Contraindications:

Erosive-ulcer damages of a gastroenteric tract in an exacerbation stage, an “aspirin” asthma (acute attacks of dyspnea caused by reception of acetylsalicylic acid), the expressed infringements of function of liver and kidneys, pregnancy, breast feeding, hypersensibility to the preparation.

It is not recommended to apply Feldene simultaneously with acetylsalicylic acid and other nonsteroid antiinflammatory preparations.

Special indicatings:

To patients with infrimgements of function of gastrointestinal tract, after extensive surgical interventions, in advanced age, to the patients with a bronchial asthma and chronic bronchospasmodic diseases in the anamnesis it is necessary to supervise cellular structure of blood, function of kidneys and a liver. At women reduction of contraceptive effect of intra-ulterus contraception is possible due to influence of Feldene on myometrium tonus. The recommendations on application for children are not developed at the moment. During treatment it is necessary to exclude the application of ethanol. During treatment it is necessary to be careful at driving and employment by other potentially dangerous kinds of activity, demanding raised concentration of attention and speed of psychomotor reactions.

Ponstel

March 4, 2008

Pharmacological action. Possesses analgesic (anesthetizing), febrifugal and anti-inflammatory activity and as anti-inflammatory means surpasses acetylsalicylic acid by activity. By analagesic activity the Ponstel is equivalent to butadiene and surpasses acetylsalicylic acid, and by febrifugal action is equal to these preparations.

By the mechanism of action the Ponstel is close to other not steroid anti-inflammatory substances. By the ability to oppress biosynthesis of prostoglandins (prostaglandins - biologically active substances developed in an organism. Their role in an organism is extremely multisided, in particular, they are responsible for occurrence of pains and hypostases in the place of an inflammation) takes an intermediate place between butadiene and indometacin.

The indication to application. Ponstel is applied at rheumatism, a nonspecific infectious polyarthritis (an inflammation of several joints), arthralgias (articulate pains) and muscular pains, neuralgias (pains, extending along a nerve), at headache and toothache, etc. and as febrifugal at various feverish conditions (the raised body temperature).

Contra-indications
The mefenamic acid is counter-indicative at a stomach ulcer and ulcer of a duodenal gut and at inflammatory diseases of a gastroenteric tract.
It is necessary to be careful at prescription of a preparation to the patients with diseases of haematogenic organs and kidneys.

Collateral action
The preparation is usually well processed (sometimes it is appointed at bad bearableness of acetylsalicylic acid), however it can cause nausea, pains in a belly cavity, a diarrhea. For prevention of these phenomena the preparation is accepted after meal. It is recommended to wash down the tablets with milk. The allergic phenomena (a skin itch, a rash) are possible in some cases.

From nervous system and sense organs: headache, tremor, spasms.
From cardiovascular system and blood (blood formation, hemostasis): hypokaliemia, palpitation.
From a respiratory system: paradoxical bronchospasm
From the musculoskeletal system: pains in joints.

Allergic reactions:
seldom — feeling of constraint in breasts, itch, nettle-rash, edema of the eyelids, lips, the face, mucous membrane of an oral cavity and/or a throat.

Interaction:
Shows antagonism with beta-adrenoblockers (including with cardioselective).
Derivatives of xanthine, glucocorticosteroids and derivatives raise the risk of development hypokaliemia.
Inhibitors of МАО and tricyclic energizers increase the risk of development of by-effects from cardiovascular system.
Combinable with inhalation glucocorticosteroids and cromoglycidic acid.

Overdose:
Symptoms: headache, tachycardia.

The information that the popular antiasthmatic the preparation salmeterol (Serevent) raises the risk of life-threatening attacks at black-skinned Americans have led to that the company-manufacturer has brought additional cautions in the summary to a preparation. However, the researchers suppose, that this danger is generated not so much by ethnic differences, but by a smaller availability of medical aid, the worst control of an asthma and more rare application inhalation of steroids in this group of the population. They approve, that it is possible to apply Serevent and preparations similar to it safely for improvement of the control of symptoms, but only in a combination with inhalation hormones.

Naproxen

February 1, 2008

Generic Naprosyn - Naproxen

Chronic pain is a painful syndrome, which during the certain period of time brings discomfort to the patient. Duration of this period of time is a conditional value, that does not allow to define precisely the moment when the acute pain transforms into a chronic pain. The chronic pain is a final result of a lot of physiological, psychological and social processes. These biopsychosocial components of a chronic pain interact and influence each other. The patient with a chronic painful syndrome often ceases to pay attention to a pain, starts to perceive it as something due and inevitable and continues to be engaged in the normal daily activity. In many cases the patients with a chronic painful syndrome on the contrary become excessively subordinated and dependent: they demand greater attention to themselves, feel seriously sick, start to rest more and relieve itself of responsibility for performance of some duties. It interferes the process of recovery and delays it.

Naproxen belongs to the group of preparations, which refer to non-steroid anti-inflammatory means (NAIM). These preparations are often used for treatment of an arthritis. Application of NAIM may have serious consequences for health down to a lethal result in consequence of a gastric or intestinal bleeding. The bleeding may arise at any time without any warning symptoms. Some doctors consider, that for the people older than 70 years there is better to begin treatment by preparations of this group from half of usual therapeutic doze.

Naprosyn possesses the expressed anesthetizing, anti-inflammatory and febrifugal properties. The active substance Naproxen, a non-steroid derivative of propionic acids, suppresses synthesis of prostaglandin. The prostaglandin plays an essential role in genesis of inflammation, pain and temperature reaction.

Naproxen is applied for reduction of a pain of small or average intensity, symptoms osteoarthritis, rheumatoid arthritis, ankylosing spondylitis (Bechterew’s disease), tendonitis, bursitis and strong attacks of gout. As a rule, if you are older than 60 years you should accept smaller dozes of a preparation, than it is usually appointed, especially if the function of your kidneys is lowered.

Indications

Symptomatic treatment of acute and chronic pains at inflammatory-degenerate diseases of rheumatic group (rheumatoid arthritis, arthrosis, ankylosing spondylitis), extra-articular rheumatism (bursitis, tendonitis, lumbago, etc.), dysmenorrhea, surgical operations and traumas, acute attack of gout.

By-action.

Naprosyn in the specified dozes is well transferred both at short-term and long-term application. There can be unpleasant sensations in the field of the top department of a gastroenteric path (pressure, pain, heartburn), and also a headache, dizziness, hidrosis. Skin rash appears much less often.

Contra-indication.

Absolute contra-indications are not revealed. It is not recommended to prescribe a preparation to children in the age under 16 years, and also to the patients, who had gastroenteric bleedings in the past; to pregnant women, to women during a lactation and to the patients with infringement of function of a liver, with cardiac insufficiency and with an allergy on anti-inflammatory preparations (especially on acetylsalicylic acid).

Do not drink alcohol. It irritates mucous coat of a stomach and raises the risk of a gastric bleeding.

If you have the symptoms reminding symptoms of a flu (fever, raised temperature, muscular pain, other pains) before or simultaneously with the advent of rash, address to the doctor immediately. It may suggest serious negative effects.

This preparation can cause dizziness when getting up from sedentary or lying position. When getting up from a bed it is necessary to let the legs down and to stay in such position for some minutes, and then to get up carefully. When getting up from a chair it is necessary to get up slowly and to stand for some time beside the chair until you are convinced, that at you don’t feel giddy.

If you are going to have a surgical operation, including treatment of teeth, inform the doctor that you accept the given preparation.

Accept with food for reduction of irritation of a stomach. Wash down with a full glass of water (8 ounces). Do not lay down within the next 30 minutes after reception.

If you have missed the reception, it is necessary to accept a preparation as soon as you have recollected it, however if the time of reception of a following doze has already almost come, miss reception. Do not accept double dozes.

Call a doctor if you constantly, during long enough time, have: a constipation, a diarrhea, dizziness, drowsiness, infringements of a rhythm of heart, a sleeplessness, a condition of delirium, muscular weakness, solar burn, hidrosis, weakness.

Motrin

January 31, 2008

Motrin ( Ibuprofen ) — a non-steroid anti-inflammatory preparation. Renders anti-inflammatory, analgetic and febrifugal action. The mechanism of action is connected with inhibition of synthesis of prostaglandins.
Preparation inhibits the mobility of leukocytes and phagocytosises. Motrin influences immune processes, breaking recognition by receptors of lymhocites and thymocytes of specific signals.
The anti-inflammatory effect comes by the end of the first week of treatment. The preparation considerably (down to disappearance) reduces a painful syndrome, including pains in joints in rest and at movements, increases volume of movements, reduces morning constraint and a swelling of joints.
Indications:

  • rheumatoid arthritis, including the juvenile;
  • osteoarthrosis;
  • psoriatic arthritis;
  • ankylosing spondylitis;
  • acute bursitis, tendinits, radiculitis, scapulohumeral periarthritis;
  • painful syndrome of the easy and moderate expressiveness (at traumas of soft fabrics, algodismenorrhea); head- and toothache;
  • superfluous menstrual loss of blood, including at the patients applying intrauterine contraceptive means.

Collateral action:
From gastrointestinal tract and a liver: a pain in a stomach, nausea, vomiting, diarrhea, anorexia, erosive-ulcer defeats of a gastrointestinal tract. There are rare bleedings from various departments of a gastrointestinal tract. Cases of increase of activity of hepatic transaminases, jaundices are described.
From urination system: it was informed about the cases of insignificant hematuria, proteinuria and seldom of nephrotic syndrome. Cases of acute interstitial nephritis, renal insufficiency at long application of a preparation are described.
Allergic reactions: nettle-rash, Quincke’s edema, bronchospasm.
From peripheral blood: seldom — decrease of haematocrit and hemoglobin.

Contra-indications:
— raised sensitivity to ibuprofen and other NSAID in the anamnesis;
— “aspirin triad”;
— ulcer of a stomach and of a duodenal gut.

Special instructions:
To patients, who in the anamnesis have the instructions on pathology of the gullet, stomach and duodenal gut, Motrin is appointed only under condition of careful supervision.
At presence at patients of the expressed infringement of function of kidneys the regular control of parameters of their function and, possibly, decrease in a doze of preparation is necessary.
The greatest risk of development of complications from function of kidneys at purpose of a preparation is also for patients with diseases of kidneys in the anamnesis, cardiac insufficiency, and dysfunction of liver, at the patients, accepting diuretics and at elderly patients. At occurrence of attributes of infringement of function of kidneys the preparation should be cancelled, then the condition usually comes back to the initial (before treatment) level.
At occurrence of infringements of sight the preparation should be cancelled, and the patient should visit the oculist, including definition of the central fields of vision and colour perception.
Motrin should be carefully applied to the patients having infringements from curtailing system of blood of a various degree of expressiveness as the preparation is capable, similarly to other not steroid anti-inflammatory preparations, to inhibit aggregation of thrombocytes and to extend the time of a bleeding (though within the limits of norm).
Because of febrifugal and anti-inflammatory effect of a preparation, the differential diagnostics of a painful syndrome can be complicated.
Controllable clinical researches of application of Motrin at children were not conducted.
It is not recommended to apply a preparation during pregnancy because of possible adverse influence on a fetus. Reception of NSAID may lead to overbearing of pregnancy and lengthening of childbirth.
Prescription of Motrin during lactation is not recommended.

Overdose:
Symptoms: frustration from gastrointestinal tract, a rash, a headache, nystagmus, infringements (illegibility) of sight, muscular twitchings, oppression of CNS, convulsive attacks, apnoea, bradycardia, hypotonia, renal insufficiency, coma.
Treatment. In case of acute overdose it is necessary to release a stomach by its washing or an induction of vomiting. Careful supervision over patients with developed depression of CNS, convulsive attacks is necessary. As the preparation has sour reaction and is deduced with urine, application of alkalis is theoretically expedient. Application of the activated coal reduces absorption of Motrin. In the case of necessity to conduct the correction of acidosis and a bradycardia, the control over a condition of hemodynamics, breathe.
It is recommended to observe the patients during several days because of a possibility of bleeding or ulceration in the gastrointestinal tract.
